I also acknowledge that the Dispute Resolution Policy contains notice of the mandatory company policy of Yee & Associates, P.C. (“Yee”) requiring that covered Claims or disputes, including but not limited to Claims for personal or bodily injury or death whether sustained on or off the job and employment-related Claims, that cannot otherwise be resolved between Yee and me must be submitted to final and binding arbitration for determination. I understand that by receiving the Dispute Resolution Policy and becoming employed or continuing my employment with Yee, I am accepting and agreeing to comply with the dispute resolution requirements in the Dispute Resolution Policy. I am giving up my right to a bench trial before a judge or a jury trial on covered Claims. I understand that Yee is also accepting and agreeing to the dispute resolution requirements in the Dispute Resolution Policy. All Claims covered by the Dispute Resolution Policy brought by persons directly or indirectly claiming through or on behalf of me, including my estate, heirs, spouse, children, parents, siblings, executors, administrators, representatives, beneficiaries, guardians, attorneys, or assigns, are also subject to the Dispute Resolution Policy, and any decision of an arbitrator will be final and binding on such persons and Yee.
